I think both had planned and had been planning are correct from the point of view of sense but had been planning is the best I mean the best of all four choices if we looked at the sentence from the point of view of tense as it indicates that the action of planning was going on when the action of illness happened which means the action of planning was interrupted by the action of illness . As for number 6 was is definitely the correct and the best answer because (v. to be + to + inf.) means should have or was ordered to ...
