"I'm Cairen" can only mean he behaves like Cairens if it (the sentence) is used as a main clause after "When"clause - like the following:
When I am in Cairo, I'm a Cairene. I don't turn either.
(Note the fullstop here)
But the writer wrote it as follows:
When I am in Cairo, I'm a Cairene , I don't turn either. I hardly hear a siren there.
(See that it is a parenthetical sentence to give additional information)

Without the parenthetical sentence it reads:
When I am in Cairo, I don't turn either.
