You have presented such helpful ideas. I do most of that and here is my point of view.
What I've noticed in most of the colleagues` notes is that they separate between the tape script and the vocabulary. However, the recent trend has stressed CONTEXTUALISATION. Words cannot be separated from the context and put on a separate list. In the A-Level books now, whether for IG or American Diploma, the vocabulary is shaded in a different colour which is usually bright and stunning. (YELLOW is the most used, but unluckily it won't appear colourful but dark and dull in our copies to the students). Therefore, I write the tape script and on the same page if possible all the vocabulary of that certain text. Everything concerning the text is on one page. We normally have 2 main texts. That makes the same amount of pages or even less if you just put the vocabulary on long lists and then the text, or vice versa. But the benefits are great, both teacher-wise and student-wise. We teach a language, not only words. The ideas of using bold, italics, shading and even underlining would fit very well here.
